Frederick Chase (Service no 521) was born in Norwich, England and served 3 years with 3rd Battalion, Norfolk regiment before he came to Australia.

At the time of enlistment Frederick was single, 35 years of age, resided in Helensburg, NSW and was employed as a miner.

Frederick embarked on HMAT A19 - Afric with the 1st Battalion, D Company on 18 October 1914.  He arrived in Egypt in December 1914 and as part of the first contingent arrived at Gallipoli, Turkey on 25 April 1915.

During the first days of fighting Frederick was wounded in action and was evacuated to hospital on 29 April 1915.

Frederick was in hospital in Egypt and transferred to England for further treatment of a wound to his face.  His left eye was removed and an ocular prosthesis fitted before he was returned to Australia in January 1916.

He was medically discharged on 22 May1916.

In 1917 he married Olga Nelson and together they had 3 children.
