Samuel Pollack (Service No 4570) was born in Bulli, NSW.

At the time of enlistment Samuel was single, 24 years of age, employed as a miner and resided in Corrimal, NSW.

Samuel embarked on HMAT A71- Nestor with 19th Battalion, 11th Reinforcements on 9 April 1916.  He arrived in England on 7 June 1916 and joined 45th Battalion in France on 29 October 1916.

Samuel was admitted to hospital on 28 December 1916 and repatriated to England. 

Samuel was returned to Australia in August 1917 and discharged as medically unfit on 28 March 1918.

In 1925 Samuel married Jessie Reid in Gosford, NSW.
